In addition to learning more about Dr. Harish, you can use this site to learn about different Asthma, Allergy and Immunology conditions by checking the articles that we collected for you in our "Patient Education" section, or for the most up-dated information, by checking the links provided to you inside "Patient Resources". For your first visit with us, please complete our New Patient Form. When filling out New Patient Forms, please be aware that it is not necessary to create a patient login and password. Please print out the forms and bring them to the office at the time of your appointment. Unfortunately, we are not able to retrieve the paperwork if you fill it out online. The Patient Login, and Create An Account, on the top left, are optional features. You are welcome to login and check it, but please note that any form that you fill out through this feature can not replace our office''s new patient form which was created specifically for your visit with Dr. Harish.
